§ 92.09. BURIAL PERMITS.

  • (A) No interment shall be allowed except upon presentation to the Cemetery Superintendent or designee of a written permit issued by the Clerk-Treasurer.
    (B) Prior to the issuance of a permit the following data is required: Name of deceased, sex, race, cause of death, name of parents or kindred, owner of lot, lot number, section, division and date of burial.
    (C) The Clerk-Treasurer is directed and authorized to provide and purchase all necessary records and forms in connection herewith. An original Indiana Burial Permit must accompany all bodies. No interment will take place without this permit.
